Heartland Health Centers – Wilson
Heartland Health Centers is a federally qualified health center serving Chicago’s north side and nearby suburbs. The clinic network is open to the entire community, including the uninsured as well as those who have Medicaid, Medicare, or private insurance. In addition to the community centers, they also offer school-based health centers and integrated care sites located within agencies providing mental health and substance abuse services.
The Wilson community health center is located on West Wilson Avenue in the heart of the Chicago’s Uptown neighborhood. The Wilson center provides primary care and mental health services to patients who are uninsured, underinsured, immigrants, and refugees.
